Medications Requests

For any medications requests where you are requesting a current, repeat or recently prescribed medication, please email the medications management team using the prescriptions email. 

For urgent medication requests, please email the medications management team and type 'URGENT care home request' in the email subject. Please include the patient's name and date of birth and the medication you are requesting. This must be emailed before 12:30pm to ensure a same day response.
