The feedback you share will help your surgery team and PPG to identify areas where improvements can be made and to assess the current quality of services based on patient experience.

The survey is being hosted by Healthwatch West Sussex in order that there is independent and unbiased analysis of the insight shared by patients. Anything you share will remain anonymous.

You give your views freely and all information will be held confidentially, by Healthwatch West Sussex.
