Practice pharmacists are part of the general practice team. They handle medication queries, help the GPs with prescribing repeat prescriptions, and reconcile repeat prescriptions when hospital letters or discharge information is received. They run medication review clinics for patients who have a variety of long term conditions, patients who may be on complex medication routines, or those who need help to take their medication correctly. The pharmacists are a point of contact for both patients and the St Lawrence team for medicine information. They aim to improve the safety and effectiveness of our prescribing with regular monitoring and review.
